There are two versions of the Entry Form as follows:Both versions include areas to enter the following data on the front of the sheets:
- Level I: Competitive Category
- Level II: Recreation Category
Team Name | Captain and Member Information | Entry Fee Information |
Brunch Reservation | and Baillie Birdathon.The back of the forms contain the Waiver Form. Because these forms have to be signed and returned before May 8th. 2003, you need to obtain the official Taverner Cup Information Package, if you plan to participate.
The Level I: Competitive Category Entry Form also has Sponsorship information for you to fill out.
